Immunization Requirements & Faulkner County Health Department Immunization Clinic Hours
All students entering the seventh grade will require proof of having had a Tdap (Tetanus, Diptheria, Pertussis) vaccine. If your child did not participate in the school Tdap clinic in May, then your child will need to provide proof of having had the Tdap vaccine within the last 2 years. Please turn your shot record into the school office before the first day of school.
The Faulkner County Health Department can make appointments up to 5 days in advance for immunizations. For same day scheduling please call at 8:00 on the day the appointment is needed. The telephone number is (501) 450-4941.

If you are new to the Conway School District and will have students attending CSMS, please call the school at 329-2782 for registration information. The counselors will return the first week of August. The 8th Grade Counselor is Belinda Claunch. The 7th Grade Counselor is Jerry Johnson. They will be happy to assist you through the registration process.

Things to Know...
* Students are expected to wait in the designated area if they enter the building before 8:10 a.m. Students who arrive at school by bus before 8:10 a.m. should wait in the designated area. No student is allowed in any other part of the building without a pass. When students are dismissed from the designated area, they are to go to their locker and directly to homeroom. The homeroom teacher will give the student a pass if they need to go elsewhere.
* During inside and outside lunch periods, students are not allowed to roam the building. Any student in the hallways during lunch must present a pass.
* Eating outside in the courtyard is a privilege. The rules for the courtyard are similar to the rules for eating inside. 1) Once you are seated, you must ask permission to leave your seat for anything. 2) If a table is full you can sit along the wall. Do not block the doors or walk-ways. If this becomes a problem we will have to limit you to only sit at the tables. 3) Pick up your trash. If you drop something on the ground pick it up. 4) Do not pop your lunch bags. 5) Do not throw food. If you choose not to follow the rules you will be asked to eat inside. 6) No food or drink is allowed on the hill.
* If a student is caught with gum, candy, food or drinks in the library, that student will automatically have detention. Please understand there will be NO warnings.
* People listed on your student's information sheet are the only ones who can check your child out of school unless prior permission has been granted by the principal.
* If your child rides a bus, that is the only bus your child may ride. No bus passes will be given to students who just want to go home with a friend for the day.
* Students need to be picked up after school school by 4:00 pm.
* Please bring all excuses (Doctor, Dentist, court, counseling) to the office.
* The State of Arkansas furnishes textbooks for all students. Students are held responsible for these books. Charges for lost or damaged books will be collected before the student's teacher will issue a new book. If a lost textbook is found, the student will receive a refund for the payment. The school does not allow students to share books. Students are asked to cover all textbooks to help minimize damage.
* CSMS provides calculators in math classes for students' use. Students who lose or intentionally damage or destroy school calculators must pay the replacement cost.
* From the time of the first bell until after the last bell, students are forbidden from using cell phones, any paging device, beeper, or similar electronic communication devices, cameras, as well as MP3 players, Ipods, and other portable music devices. Exceptions may be made by the building principal or his/her designee for health or other compelling reasons.
Students should not bring large sums of money to school and should NOT leave money or valuable items in the lockers. If a student must bring a large sum of money to school, he/she may speak to office personnel and arrange to leave the money in the office.
* The school is not responsible for lost or stolen articles. All items brought to school should be marked with the student's name (especially shoes, jackets, and binders). Students should bring "found items" to the office. CSMS will make a good faith effort to help students locate lost or stolen items which are deemed necessary for educational purposes.
* The coke machines do not take 5 dollar bills. Students must not put them in the machines because they will not get them back or get change from them.
* If you are moving to another school, your student is to formally withdraw from school here at CSMS. This means that you must be in touch with the appropriate counselor at least a couple of days prior to your departure. Even if you are just moving across town to the BCMS district, you still must follow the proper procedures. All books must be turned in to the correct teachers and all fines paid.
